If you require any vaccinations relating to foreign travel then please complete the travel questionnaire. We will then review you medical records and advise you whether you require further immunisation for the area you are travelling to.  Please contact the surgery one week after completing the questionnaire to be informed if you need to see the practice nurse.

Please enquire about travel vaccinations as early as possible - at least 6 weeks before you travel - as an appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

To help us offer the appropriate advice, please fill out the online form before coming to see the nurse.

If you are going away within a month then please make an appointment to see the practice nurse as soon as completing the questionnaire as there can be a wait to see the nurse.

If you are going away in over a months time then please telephone reception one week after completing the questionnaire and they will be able to advise you what vaccinations you require.
